DS6, has Samus ever officially appeared outside of her uniform, aside from the death animation in Prime?

Well, it wasn't really a death animation...
And as for Samus appearing outside her uniform, I think this was the only time. I'll ask around, and see if I'm wrong.

What do you call it when your health hits zero, your suit explodes, and there's a brief image of the hero contorted backwards in her underwear?

You must really be confusing the game Kas. That never happened in the ending video...
She escaped on her ship as the crater exploded, and she takes off her helmet at that scene..

Have you played Metroid Prime for the GBA? I have not beaten it yet, but I'm referring to the death animation that happens every time you run out of health points throughout the game.

Oh, I thought you were referring to Metroid Prime for Gamecube. Metroid FUSION is the GBA version.
BTW, I asked around, it seems Samus' head is shown in all the games except Metroid II.
Edit: Actually, she appears with her helmet off in all the games.

Does she?On Metroid Prime on gamecube i don't think she takes it off at all.Personally i think it's a failure of a game but it has good graphics on the gamecube.

Lorenzo, you have to beat the game in a certain amount of time to unlock the video, which shows her taking off the helmet.
